Question 1
What is the best course of action?

Platelet transfusion

Oral prednisolone

IV hydrocortisone

Intravenous immunoglobulin

Observation with regular blood counts

Answer
Observation with regular blood counts.
It seems that the underlying cause for bleeding is (ITP) idiopathic thrombocytopenic purpura, since the condition is preceded by URTI and no other abnormal clinical findings. The FBC is within normal limit apart from mild thrombocytopenia, the clotting screen is normal so this excludes the coagulopathy as a cause for bleeding. So observation is the correct action because ITP is a self limiting condition and we have to interfere in special occasions when there is life threatening bleeding or when the platelet count is severely depleted i.e. below 20000/mm3.
